  • Did You Get an Identity Confirmation Quiz Letter?

    With income tax fraud, identity theft, and data breaches on the rise, the Michigan Department of Treasury is increasing security measures. The identity confirmation quiz is one of the tools the Department has implemented.


    If you did not receive a letter that contains a Quiz ID number, you are not authorized to use this option to take the identity confirmation quiz or report the return as suspicious.

    If you received a quiz letter from the Michigan Department of Treasury in response to a tax return and/or credit you filed or authorized someone else to file on your behalf, you will need to complete the identity confirmation quiz. You are not required to create an account. Simply click the “Take the Quiz” button to start the quiz. Once you have passed the quiz, the Department will continue processing your return. It is important to print your confirmation page and keep it with your tax records.

    If you received a quiz letter from the Michigan Department of Treasury and did not file or authorize someone else to file a return on your behalf, click the blue button labeled “Identify a Return as Suspicious” in order to report the return as suspicious.

    If you cannot successfully complete the identity confirmation quiz, the Department requires all of the following documents from you and your spouse (if applicable) that are associated with your return:

    • Copy of driver’s license or Government issued picture ID
    • W-2s
    • SSA-1099s
    • 1099s
    • K1s

    Attach a copy of the letter you received to the front of your response and mail to:

    Review of your mailed response may take up to 16 weeks.

  • To login and take the identity confirmation quiz, you will need:

    1. Quiz ID number from your letter, and
    2. Refund amount from the return year referenced in your letter, from your Michigan Individual Income Tax Return (MI-1040),
      If you filed a Michigan Homestead Property Tax Credit Claim (MI-1040CR), and no MI-1040 return was filed, use the amount on line 44 from the MI-1040CR.
