The Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems issues licenses to Substance Use Disorder (SUD) Programs.
A "Substance Use Disorder Services” or “Program” is a provider that offers substance use disorder prevention services or treatment and rehabilitation services, or both.

To be state licensed as an SUD program, a provider must submit an application for state licensure. After confirmation of the necessary approvals, a pre-licensure inspection will be conducted by the State Licensing Section. If the applicant is in compliance with the Code and applicable rules, a license will be issued and a post-licensure inspection will be conducted. Once licensed, the department will conduct routine inspections and complaint investigations as required.

$500 annual license fee.
