The Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems issues licenses to Hospice Agencies.
A “Hospice Agency” is a health care program that provides a coordinated set of services rendered at home or in outpatient or institutional settings for individuals suffering from a disease or condition with a terminal prognosis.
A hospice agency that is state licensed can be federally certified to participate in Medicare/Medicaid.

To be state licensed as a hospice agency, a provider must submit an application for state licensure. A pre-licensure inspection will be conducted by the State Licensing Section. If the applicant is in compliance with the Code and applicable rules, a license will be issued and a post-licensure inspection will be conducted. Once licensed, the department will conduct routine inspections and complaint investigations as required.

$500 annual license fee. There is a $2,000 application fee for initial licensure.
