The Materials Management Division within the Michigan Department of Environment, Great Lakes, and Energy (EGLE) issues Site Identification numbers to customers that will be generating hazardous waste, used oil, or universal waste; transporting hazardous waste or liquid industrial by-product; or receiving hazardous waste, liquid industrial by-product, or universal waste for treatment, storage, and/or disposal.
NAME OF NOTIFICATION AND OTHER NAMES FOR SITE IDENTIFICATION NUMBER:
STATUTORY AUTHORITY: Part 111, Hazardous Waste Management (Part 111) or Part 121, Liquid Industrial By-Products (Part 121) of Michigan's Natural Resources and Environmental Protection Act, Public Act 451 of 1994 (Act 451), as amended. Subtitle C of the Federal Resource Conservation and Recovery Act, (RCRA).
APPLICABLE REGULATION: Part 111, Section 11153; Part 111 Rules R 299.9101 - R 299.11107; and Part 121.
SUMMARY OF NOTIFICATION PROCESS:
Applicability (activities that require a site identification number): Any customer who is generating hazardous waste for disposal, transporting hazardous or liquid industrial by-product, or receiving hazardous or liquid industrial by-product for disposal needs to obtain a site identification number.
Pre-Application Requirements: N/A
Application Submission Requirements: There is a blank form number EQP 5150 that is required to be filled out to apply for the site identification number or update the information.
Procedures and Time-Frame for Obtaining Site Identification Number: If this is an initial application for a site identification number, you submit an EQP 5150 and $50 (paid either by check or online under the heading State EGLE Site ID Fee). If paid online, you can submit the form and the payment receipt by fax to 517-335-0564. If paid by check, you need to submit to the Cashier's Office, address listed on the form.
Once a complete EQP 5150 and the $50 fee is received, a site identification number is issued.

Fees: Fifty dollar fee for an initial notification. Subsequent notifications are free.
