Correct a Death Record
To correct or change a Michigan death certificate you must apply through the mail. Please complete the applicable application below and mail to the address on the top of the application along with a copy of your photo identification, application fee and required documentation. If there are any problems with your order or more documentation is needed, we will contact you. Please see our FAQ’s for required documentation.
The following forms are used to correct a Michigan death record or to verify whether or not a change was made to a death record. Please review the following list of applications to correct or change a Michigan death record.
In order to make a correction to a Michigan death record, you need to submit an official application. The application must include the correct payment and in most situations, appropriate supporting documentation. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation required, and the appropriate fee payment. Application for next-of-kin or funeral service licensee. - Photocopy of an Application to Correct a Michigan Birth or Death Record or Request a Michigan Birth Record
Application to verify whether a change has been made to a Michigan birth or death record or if a Michigan birth or death record has been requested. If a change is found, you will receive a copy of the application and a letter from the Changes Unit. If a change is not found, you will receive a letter indicating so. - Physician Application to Correct Michigan Death Record before 1/1/2004

PHOTO IDENTIFICATION (OR THESE ALTERNATE FORMS OF ID) IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN DEATH RECORD.

If you need to correct a Michigan marriage record, please contact the Circuit Court in the county where the license was obtained.
*** Please note, if you need to correct a record of a death that occurred in another state, you should contact that state's vital records office.
