MOSAIC Hospital and EMS Agency Report Templates

The MOSAIC program and its partners are working together to develop systems of care that support quality improvement across the continuum of stroke care. The following reports are used to help the MOSAIC team and its partners compare the performance of a reporting hospital or emergency medical service (EMS) agency with its peers.

Hospital-Level Reports

  • Quarterly Performance Measure/Benchmark Report Template (rev. Jan. 24, 2020)
    This quarterly report helps MOSAIC and a reporting hospital gauge the hospital's compliance with 10 MOSAIC consensus performance measures. It creates a point-in-time snapshot that can be used to compare the hospital's performance for the reporting quarter with the performance of all MOSAIC hospitals during that same quarter. The MOSAIC team uses this report to help develop strategies to further improve and maintain the program's performance measures.
     
  • Quarterly Comprehensive Stroke Centers (CSC) and Thrombectomy Stroke Centers (TSC) Performance Report Template (rev. Feb. 16, 2020)
    This quarterly report helps MOSAIC and a reporting hospital evaluate the performance of the hospital's CSC measures, compared to the performance of all participating CSCs/TSCs across Michigan. Each report highlights the performance measures with the greatest opportunity for improvement so the hospital can work with MOSAIC to develop strategies to focus efforts on improving and maintaining those measures.
     
  • Annual Performance Measure/Benchmark Report Template
    This annual report helps MOSAIC and a reporting hospital gauge the hospital's compliance with 10 MOSAIC consensus performance measures. It creates a point-in-time snapshot that can be used to compare the hospital's performance for the reporting year with the performance of all MOSAIC hospitals during that same year. The MOSAIC team uses this report to help develop strategies to further improve and maintain the program's performance measures.
     
  • Annual Hospital Data Scorecard Template (rev. Feb. 16, 2020)
    The primary goal of the MOSAIC program is to improve the quality of care and outcomes for patients hospitalized with stroke and TIA in Michigan. This scorecard is designed to help hospitals identify any quality improvement opportunities that may exist. It presents the hospital's compliance with 14 consensus performance measures on both a quarterly and annual basis and compares the hospital's compliance scores with those of other MOSAIC participating hospitals and also with Coverdell hospitals nationwide.
     
  • Annual MOSAIC Data Quality Report Template (rev. Feb. 16, 2020)
    This annual report helps MOSAIC and the participating hospital ensure the quality and reliability of data. It encompasses a hospital’s patient discharge data over a selected 12-month period, comparing data from 24 (when available) randomly selected charts in the Get With The Guidelines Stroke Patient Management Tool with data from re-abstracted charts using the MOSAIC PDF form. The report provides quarterly and overall agreement rates for the hospital, as well as for all MOSAIC participating hospitals. The MOSAIC team uses this report to target a hospital's data items with low percent agreement. (MOSAIC is targeting 90 percent as an acceptable level of agreement.)
     
  • Annual EMS Arrival Mode and Pre-Hospital Notification Report Template
    The MOSAIC Program, the MDHHS EMS Section, and other partners are working to develop stroke systems of care to support quality improvement across the continuum of stroke care. As the team works to build a data linkage between pre-hospital and in-hospital stroke data, MOSAIC is looking for ways to support hospitals and EMS providers in monitoring, evaluating and improving care of stroke events from time of EMS transport to post-discharge treatment and rehabilitation. This annual report tracks the hospital arrival mode (EMS vs. private transportation) and the existence of documented pre-hospital notification, both for the participating hospital and for all MOSAIC hospitals as a whole.
     
  • Bi-Annual MOSAIC Hypertension Data Report Template (rev. Jan. 24, 2020)
    This bi-annual report tracks the number and percent of three types of stroke patients who visited a reporting hospital within the six-month reporting period: 1) those who arrived at the reporting hospital with a history of hypertension; 2) those who arrived at the reporting hospital with a history of hypertension and who were on a prescribed antihypertensive medication at the time of admission; and 3) those who arrived at the reporting hospital with a history of hypertension and were prescribed an antihypertensive treatment at discharge (not including cases with an indicated antihypertensive treatment contraindication). It creates a point-in-time snapshot that can be used to compare the demographics of the reporting hospital's stroke patients during the reporting period with the demographics of stroke patients at all MOSAIC hospitals during that same six-month period.
     

EMS Agency-Level Reports

  • Quarterly EMS Agency Linkage Report Template (rev. Feb. 16, 2020)
    This quarterly report helps MOSAIC provide feedback to EMS agencies and the hospitals they service in order to support the monitoring, evaluation and improvement of care for stroke patients from time of EMS activation to in-hospital hand-off and treatment. It creates a point-in-time snapshot that can be used to compare an EMS agency's performance for the reporting quarter with the performance of other Medical Control Authority (MCA) agencies and/or with the performance of transporting EMS agencies statewide during that same quarter.
